Dispite the other factors the crew blamed...again

If you ask anyone you know what caused the British Midland B737-400 accident at Kegworth a few years ago I guarantee that your thoughts and their answer will be along the lines of, “the accident was caused by the crew shutting down the wrong engine.” Few will mention the engine failure, the training factors involved and the human factors leading to the incorrect engine being shut down, the missed opportunities to rectify this situation and the communication breakdowns that contributed to this accident. Instead, it is categorized as pilot error. I find that sad because this conclusion does not reflect the whole story and does not fix the accident or prevent a similar occurrence……
